What is Support at Home?

Support at Home is the Australian Government's in-home aged care program that helps older people live safely and independently at home. It combines previous aged care services into one simple system, focusing on assessed needs and flexible support. Services include personal care, domestic help, nursing, and social support, all delivered by approved local providers.

Eligible older Australians can access care tailored to their individual needs, helping them maintain independence and wellbeing. Support at Home is available across metropolitan, regional, and rural areas, ensuring practical, reliable, and person-centred in-home aged care.

How to access Enrich Home Supports aged care services

How to access our services.

Apply through My Aged Care

To begin accessing aged care services with Enrich Home Supports, the first step is to apply through My Aged Care, the Australian Government's entry point for aged care support.

If you are aged 65 years or over (or 50 years or over for Aboriginal and Torres Strait Islander peoples), you can register online or by calling My Aged Care on 1800 200 422 to start your application.

Assessment of your needs

Once your application is submitted, a My Aged Care assessor will contact you to arrange an assessment. This assessment looks at your physical, emotional, and social needs and gives you the opportunity to discuss your goals, preferences, and the support you may require to continue living well at home.

The outcome of this assessment determines the types of services or funding you may be eligible to receive.

Develop your care plan with Enrich Home Supports

Following your assessment, you will receive a summary of the outcomes, including whether you have been approved for funded aged care services.

Enrich Home Supports will then work with you (and your family or carers, if you wish) to develop a personalised care plan that reflects your needs, preferences, and lifestyle. Your plan may include support such as daily assistance, nursing care, allied health services, social support, and companionship — delivered in a way that supports your independence and wellbeing at home.

When you commence services with Enrich Home Supports, we work with you to develop a personalised care plan tailored to your individual needs, goals, and preferences. Your care plan focuses on supporting your overall wellbeing and independence, and may include assistance with daily activities such as meal preparation, personal care, and household tasks.

02 · Health & emotional support

Medical, health and emotional support.

Health and wellbeing are central to the care we provide. Enrich Home Supports can assist with medication management, coordinating and attending medical appointments, and accessing appropriate healthcare professionals. Where needs are more complex, support may include nursing care and allied health services such as physiotherapy or podiatry.

We also recognise the importance of emotional wellbeing and social connection. Our services support you to stay engaged through meaningful social activities, community participation, and outings, helping to reduce isolation and support your overall quality of life.
